Physiology of Menopause
Overview of Menopause

 Menopause typically occurs in women aged 40-58 years, with an average life expectancy
of 81.2 years.

 The STRAW+10 classification is the gold standard for characterizing reproductive aging,
consisting of 7 stages: 5 before menopause and 2 after the final menstrual period (FMP).

 The term 'perimenopause' refers to the transitional phase leading up to menopause,
characterized by endocrinologic, somatic, and psychological changes.

 Premature Ovarian Failure: Menopause occurring before age 40, also known as

Stages of Reproductive Aging

 At birth, females have approximately 1-2 million oocytes, which decrease to 300,000-
500,000 at puberty.

 Only about 400-500 oocytes will be ovulated during a woman's reproductive life.

 The reproductive stages include early (-5), peak (-4), and late (-3a and -3b), with specific
hormonal profiles for each stage.

Menopause Transition Stages

 The menopause transition consists of two stages: early (-2) and late (-1) menopause
transition, characterized by changes in cycle length and hormonal fluctuations.

 The LOOP event describes a physiological phenomenon during the menopause transition
where elevated FSH leads to the recruitment of additional follicles, resulting in elevated
estradiol levels.

 The primary driver of changes in the hypothalamic-pituitary-ovarian (HPO) axis during
menopause transition is follicular depletion, which begins before birth.

leading to increased FSH levels and subsequent follicle recruitment.

Laboratory Assessments for Menopause

 FSH and estradiol (E2) should be assessed together to evaluate menopause status, as
elevated E2 can suppress FSH levels, leading to falsely normal results.

Primary Ovarian Insufficiency (POI)

 POI is characterized by hypergonadotropic hypogonadism in women under 40, affecting
approximately 1% of women by this age.

 Turner Syndrome is the most common genetic disorder associated with POI, occurring in
1 in 2,500 to 3,000 births, with 99% resulting in miscarriage.

Treatment and Management of POI

 Hormone therapy is recommended for women with POI until the average age of
menopause to mitigate long-term effects of estrogen deficiency.

 In cases of delayed puberty due to Turner Syndrome, estrogen replacement should begin
at age 12, with gradual increases to a target dose of 100 mcg.
