If a patient has a mastectomy on the right arm and dialysis
|\ |\ |\ |\ |\ |\ |\ |\ |\ |\ |\ |\
fistula on the other where would you take the BP?
|\ |\ |\ |\ |\ |\ |\ |\ |\
Thigh
Patients with findings below, which one will you see first?
|\ |\ |\ |\ |\ |\ |\ |\ |\
HR: 80 bmp |\ |\
BP: 80/50 mmHg
|\ |\
Temp: 98.9 |\
Respirations: 18 RR T |\ |\ |\
Patient with BP of 80/50 mmHG |\ |\ |\ |\ |\
What are Vital Signs?|\ |\ |\
Temperature
Pulse
Respirations
Blood Pressure |\
O2
Saturation
Pain
Two Step BP measuring
|\ |\ |\
1. Palpate the *BRACHIAL* or Radial artery, inflate the blood
|\ |\ |\ |\ |\ |\ |\ |\ |\ |\
pressure cuff until the pulse is no longer felt, and then continue
|\ |\ |\ |\ |\ |\ |\ |\ |\ |\ |\ |\
to inflate 20-30 mm Hg more: this is the maximum pressure
|\ |\ |\ |\ |\ |\ |\ |\ |\ |\ |\
inflation.
2. Deflate the cuff quickly.
|\ |\ |\ |\
3. Now, you can start blood pressure so place the bell of the
|\ |\ |\ |\ |\ |\ |\ |\ |\ |\ |\ |\ |\
cleansed stethoscope over the brachial artery using a light touch
|\ |\ |\ |\ |\ |\ |\ |\ |\ |\
, and complete seal.
|\ |\
4. Inflate the cuff to the maximum pressure inflation number.
|\ |\ |\ |\ |\ |\ |\ |\ |\
5. Open the valve slightly.
|\ |\ |\ |\
6. Deflate the cuff slowly and evenly at about 2 mm Hg per
|\ |\ |\ |\ |\ |\ |\ |\ |\ |\ |\ |\ |\
second.
7. Note the points at which you hear the first appearance of
|\ |\ |\ |\ |\ |\ |\ |\ |\ |\ |\ |\
Korotkoff sound (systolic blood pressure), and the last Korotkoff
|\ |\ |\ |\ |\ |\ |\ |\ |\
sound before it goes silent (diastolic blood pressure).
|\ |\ |\ |\ |\ |\ |\
Blood Pressure (BP)
|\ |\
Normal : 120/80 BP |\ |\ |\
Hypertension: 140/90mmHg |\
Hypotension: 90/60 mmHg |\ |\
Temperature
Normal: 97.5 - 99.5 F |\ |\ |\ |\
Hyperthermia: above 100.2F (significant is 101.3F) |\ |\ |\ |\ |\
Hypothermia: below 95F |\ |\
Where do you check Temperature?
|\ |\ |\ |\
-*Tympanic (eardrum) temp approximate core body temp*
|\ |\ |\ |\ |\ |\
-Rectal temp about 1 degree higher than oral
|\ |\ |\ |\ |\ |\ |\
-Axillary temp about 1 degree lower than oral
|\ |\ |\ |\ |\ |\ |\
-Temporal Artery over skin of forehead |\ |\ |\ |\ |\
Apical Pulse |\
5th intercostal space, mid-clavicular line for 1 min
|\ |\ |\ |\ |\ |\ |\
*checked before given any medication especially BP *
|\ |\ |\ |\ |\ |\ |\
Pulse Rate (bmp)
|\ |\
Normal: 60-100bmp |\
Bradycardia: <60bmp |\
Tachycardia: >100bmp |\
