Introduction
According to Healthy People 2030 (2020), mental illnesses impact persons of all ages and
races/ethnicities; however, some groups are disproportionately affected. According to estimates,
just half of mental health patients receive treatment. Thus, optimizing patient outcomes and
promoting holistic well-being across the healthcare continuum requires effective care
coordination. Building on assessment 1, this assessment explores the complications of patient-
centered care for mental health patients. Mental health is crucial since it is a global public health
issue. This assessment incorporates evidence-based best practices and ethical concerns to
improve the care coordination plan and ensure patients receive comprehensive, culturally
appropriate, and effective therapies.
Patient-Centered Health Interventions and Timelines
Mental Health Issues
Mental health concerns are complicated and require careful treatment. Mental health
issues include depression, anxiety disorders, social isolation, and loneliness. Depression is a
common mental health disorder characterized by chronic sorrow, loss of interest, and impaired
functioning performance. Anxiety disorders encompass generalized anxiety, social, and panic
disorders. They are characterized by excessive worry, fear, and avoidance. Loneliness can cause
depression and anxiety. Lack of social relationships can have severe mental and emotional health
implications.
