reversible versus irreversible factors). An important first step is a thorough
medical, neurologic, and psychologic history. During the history, special
attention is given to reviewing the cognitive and behavioral changes that
have occurred. Family members and significant others can give important
information. Elicit information about (1) problems with judgment; (2)
reduced interest in hobbies/activities; (3) repeating questions, stories, or
statements; (4) trouble learning how to use a tool or appliance; (5)
forgetting the correct month or year; (6) problems handling financial
affairs; (7) difficulty remembering appointments; and (8) consistent
problems with thinking and/or memory.
Obtain a complete medication history, including the use of prescribed and
over-the-counter medications, herbal supplements, and recreational
substances. Ask about drugs that can impair cognition, such as analgesics,
anticholinergics, psychotropics, and sedative-hypnotics.
A thorough physical assessment is done to rule out other potential medical
conditions. For example, slow movement, rigidity, asymmetric tremor of an
extremity, and shuffling gait are suggestive of Parkinsonism. Dementia,
urinary incontinence, and ataxic gait suggest normal pressure
hydrocephalus. The neurologic assessment includes a mental status
examination or screening test. Agreement among findings from the
assessment, screening tests, and history can help confirm the presence of
dementia.
Based on history and physical assessment findings, diagnostic studies are
ordered to confirm the most likely cause and exclude other possible
conditions. The American Academy of Neurology (AAN) recommends
routine screening tests, including an electrolyte panel, liver function tests,
serum vitamin B12 level, complete
