1. HEENT exam
Head, neck, eyes, nose, throat, hair, shoulders, lymph nodes, thyroid gland,
in- cludes
trachea position, carotid arteries, jugular veins
2. Cranial
CN V- sensory of face- have pt close eyes while touch with something
nerves
evaluated in soft (ex: cotton) ask pt to clench teeth
HEENT: CN VII- symmetrical movement of face- test by having pt smile, putt cheeks,
raise eyebrows, frown, close eyes
CN XI- head and shoulder strength- press down on shoulders and ask pt
to shrug against resistance
3. Palpating occipital, post auricular, preauricular, tonsillar, submandibular, submental,
lymph nodes anterior cervical nodes, posterior cervical nodes, supraclavicular nodes
HEENT
Thyroid gland should not be visible if averaged sized
4. Examining Instruct pt to take a sip of water while you feel as it moves up with
thy- roid trachea
gland
Auscultating thyroid: if thyroid is enlarged, auscultate with stethoscope. A
bruit will be present if increased blood flow, possibly due to
hyperthyroidism
5. Trachea Inspect for deviation, masses
6. Eye exam Order:
1. Visual Acuity- snellen for far, Rosenbaum eye chart near (CN II)
2. EOMS- H test (III, IV, VI) (extra ocular)
3. Visual Fields- confrontation (counting fingers) (CN II)
4. Corneal light reflex, pupil reaction to light (CN II, III)
Looking for changes in tolerance to glare, yellowing of lens
7. Rosenbau Hold 14 inches away from face to screen for presbyopia (impaired near
m chart vision)
