The nurse practitioner plays a significant role in working with the patient and the family in assisting with care transitions. Earl et al. (2020) state that healthcare provider involvement can contribute to safe and efficient care transitions. It is the health care provider's responsibility to establish a care plan that includes patient and family education, effective communication, and transfer of information to other facilities or providers about all appropriate information regarding the patient's health status, considerations, and medications and to facilitate the continuity of care by establishing follow-up appointments (Earl et al., 2020). Identify at least TWO appropriate referrals for a client who requires a transition in the care environment. How does the NP ensure coordination of care during transitions? The appropriate referrals for a patient who requires a transition in the care environment are a social worker referral and a referral to a home health agency or skilled nursing facility.
