osis..and..treatment...To..put..their..plan..in..action,..Fielder..decided..to..focus..on..those..patients..they.

.have..classified..as..“high..risk”...High..risk..by..their..standards..includes..previously..readmitted..pati


ents..and..those..who..were..admitted..for..illnesses..such..as..congestive..heart..failure..or..pneumonia..

, eptic..Shock”.. . Sepsis..is..a..severe..infection..in..the..body..that..usually..is..caused..by..chemicals..bei

ng..released..into..the..bloodstream...Once..in..the..bloodstream,..those..chemicals..travel..to..other..org

ans..in..the..body,..which..results..in..the..bodily..systems..shutting..down..and..could..cause..death...Fi
