-foul smelling lochia -inform patient of procedure to be done
-temperature greater than 100.4 -obtain consent if needed
-tachycardia -moitor for any complications after procedure

Client Education
goal is for patient infection to heal in a timely
manner and be pain free. good hygiene techniques include changing
perineal pads from front to back
-thorough hand hygiene prior to and after
perineal care.




Potential Complications Nursing Interventions
infection worsens -administer IV antibiotics
wound does not heal -administer analgesics for pain control
-monitor temperature frequently to ensure
antibiotics are effective
-frequent vital signs
-assess pain
-observe lochia for color, quantity,
consistency
