-55 - Answer-Postoperative Management Only. When one physician performed the
postoperative management and another physician performed the surgical procedure,
the postoperative component may be identified by adding this modifier.
-56 - Answer-Postoperative Management Only. When one physician performed the
preoperative care and evaluation and another physician performed the surgical
procedure, the preoperative component may be identified by adding modifier 56 to the
usual procedure number.
-58 - Answer-Staged or Related Procedure or Service by the Same Physician or Other
Qualified Health Care Professional During the Postoperative Period.
-59 - Answer-Distinct Procedural Service. Used in non E/M procedures. Under certain
circumstances, it may be necessary to indicate that a procedure or service was distinct
or independent from other non E/M services performed on the same day. Known as the
"unbundling" modifier.
-63 - Answer-Procedure Performed on Infants Less Than 4 kg. I can be only appended
to surgical CPT code within range of 20000-69990. It should not be appended to any
CPT codes listed in the E/M , Anesthesia, Radiology, Pathology/Laboratory,or Medicine

Physician or Other Health Care Professional Following Initial Procedure for a Related
Procedure During Postoperative Period. The global period stays the same

Health Care Professional During the Postoperative Period. This modifier starts a new
global period
-27 - Answer-Multiple Outpatient Hospital E/M Encounters on the Same Date
-50 - Answer-Bilateral Procedure. Unless otherwise identified in the listings, bilateral
procedures are performed at the same session. Do not use with codes that are
inherently bilateral or contain description of unilateral or bilateral.
