General Survey - ANSWER An observation or "survey" of a patients "general"
appearance. In the office this starts from the time you call the patients name. In the
hospital this begins the moment you walk in to the patients room. "Birds eye view".
Study of the whole person, covers general health state, looks at physical
characteristics, introduction to the initial physical exam, and begins IMMEDIATELY!



General Survey Observations - ANSWER Does the patient respond? Do they make eye
contact? Smile/frown? How is their gate? Stance? How is their dress? May assess vital
signs at this time, but NOT a part of survey, that is objective patient data.



General Survey- What is the purpose of? To start gathering information and to form a
general impression of your client/patient. "People watching". However, not always
typical of a person's personality. Q: What gives you an immediate impression when you
first encounter your patient?



Physical Appearance- What does one notice? Age, sex, LOC, skin color and facial
features



Body Structure - ANSWER Stature, nutrition (weight and height), symmetry (equal on all
sides. No signs of stroke), posture, position, contour



Mobility - ANSWER Gait, ROM (range of motion)

,Behavior - ANSWER Facial expression, mood and affect, speech, dress, hygiene.



Age - General Survey - ANSWER Patient appears to be his or her stated age.



Skin Color - General Survey - ANSWER Skin tone is even. Palor? Cyanosis? Jaundice?



Physical Appearance - General Survey - ANSWER Facial features - symmetrical or
assymetrical.

Obese patient, patient with facial work, starving patient, patient who had a
stroke-paralysis of lower jaw vs Bell's Paulsy-can't even lift their forehead.

Are their movements symmetrical on face?



Level of Conciousness - ANSWER Alert, lethargic, obtunded, stupor/semi-coma, coma



Alert - ANSWER Awake and interactive. Can be confused though.



Lethargic (somnolent) - ANSWER Falls asleep. Can be easily aroused. Very drowsy.
Responds to stimulus such as names and questions but falls back asleep when stimulus
stops.



Obtunded - ANSWER Must shout or shake patient to wake them.



Stupor/Semi-Coma - ANSWER Only respond to painful stimuli



Coma - ANSWER No response to ANY stimuli. If patient is in a light coma, there may be
some reflex activity, but movement is not purposeful or meaningful.



Stature - ANSWER Height appropriate for age and genetic heritage

,Nutrition - ANSWER Weight appears normal range for height and body build. Fat
distribution is even



Symmetry - ANSWER Patient is equal bilaterally. Address symmetry and any
abnormalities.



Posture - ANSWER Erect-Normal or stooped-kyphosis, lordosis, and scoliosis



Position - ANSWER Sits comfortably with arms relaxed at sides with head turned to
examiner.



Body Build / Contour - ANSWER Arm span and body length are equal.

Long arms = marfans syndrome, short arms = dwarfism.



Kyphosis - ANSWER Spine hunched out at thoracic / cervical spine. Common in
depressed patients as well. (Osteoporosis)



Lordosis - ANSWER Lumbar spine depressed further in than normal. Protuberant
abdomin. (Usually in toddler's)



Scoliosis - ANSWER Patients spine is "S" curved. Typically off-sets hips and shoulders.
One may be higher than other



Osteoporosis - ANSWER Stooped kyphosis due to bone deterioration usually in older
women



Tripod Stance - ANSWER Patient stands this way to expand chest and breath easier.
Typically has COPD (Chronic Obstructive Pulmonary Disorder). Pt refuses to lie down?
Possible Congestive Heart Failure (CHF).

, Gait - ANSWER Base as wide as the shoulders, foot placement accurate, smooth, even
and well balanced, and associated movements equal, symmetrical and opposing arm
swings when walking.



Range of Motion - ANSWER Full, Accurate smooth and coordinated, No involuntary
movements (tics or tremors)



Abnormal Mobility - ANSWER Shuffle, drag, limp, foot drop, difficulty, tics, tremors,
issues with starting and stopping (parkinsons) limited ROM (arthritis) Parapalygic =
paralyzed from waist down. Quadrapalygic = paralyzed from neck down.



Facial Expression - ANSWER Maintains eye contact. Expressions are appropriate for
situation.



Mood and Affect - ANSWER Comfortable and cooperative with examiner. Interacts
pleasantly. Is patient anxious? Flat affect? Angry? Hostile, distrustful, emotionally
challenged?



Speech - ANSWER Articlation is clear and understandable. Abnormal? Clear?



Dysarthria - ANSWER Articulation problems



Aphasia - ANSWER Unable to understand and/or respond



Dysphonia - ANSWER Disorder of the voice



Sex - General Survey - ANSWER Sexual development is appropriate for age?

Opposite gender than stated?

Same gender?
