SUBJECTIVE DATA:
Chief Complaint (CC): In just a few words, explain why the patient came to the
clinic.
Patient states he's been having some troubling chest pains for about a month.
History of Present Illness (HPI): Patient is a 58-year-old Caucasian man who
reports to the clinic with reports of chest pain for the last month. Patient describes
the pain as in the middle of his chest, is tight, kind of like heart burn, but passes in a
few minutes. He rates the pain as a 5/10 at its worst, but currently at a zero. These
episodes of pain have occurred 3 times in the last month and while it feels similar
to heart burn, he doesn’t feel like it’s indigestion or related to food intake. Moving
around makes the pain worse or at least more noticeable
Medications: Include over the counter, vitamin, and herbal supplements. List
