P6- Explain why meeting the needs of the individuals requires the involvement of different
agencies.
Clinical Commissioning Groups
Clinical Commissioning Groups were created in 2013 by the NHS to help assess the needs of
a specific area and are overseen by the NHS England. It was created following the Health
and Social Care Act 2012. Clinical Commissioning Groups commission most of the hospital
and community NHS services in the area. Commissioning is about getting the best possible
health outcomes for the local population. As said before this involves assessing local needs.
It consists of health care professionals such as GP’s and practice nurses. Tremayne could
benefit and use Clinical Commissioning Groups to support him in his care. It will allow him to
have access to care such as counselling for his outbursts of temper. This will help him to
become calmer and may reduce the outbursts as he will have a way of dealing with it better.
Local Authorities
Local Authorities provide a range of public services in a local area. They promote
personalised care for the local community. Local Authorities provide information to disabled
individuals and their carers, ensuring that they are aware of the support options available in
the local area. Local Authorities can also assess the needs of those individuals to ensure that
they are receiving the correct care. Individuals and their carers should be able to access
advice from their local authority. They also work in partnership with organisations, such as
housing and the NHS in order to support individuals within the social care sector. This will
benefit both Tremayne and Aisha as the Local Authority has an Adult Social Care
Department where social workers work with people like Tremayne who is autistic and Aisha
who is elderly and suffers from vascular dementia. If they get assessed by the Local
Authorities and are receiving the correct care, it is likely that they both will increase their
health and wellbeing positively.
Health and Wellbeing Boards (HWB)
Health and Wellbeing Boards were introduced by the Health and Social Care Act 2012 to try
and integrate public services for health and social care. They assess the needs of the local
area and tasked with reducing inequalities and setting out strategies and approaches for the
local authorities, CCG’s and NHS England to improve decision making. These boards help
develop on building relationships with multi agencies to achieve the best possible success in
healthcare services. Health and Wellbeing Boards will be beneficial for both Tremayne and
Aisha, as it will allow them to receive the best possible care from their health care
professionals. Health care professionals will feel motivated to help and support both of
them that is in the best interest for them.
Common Assessment Framework (CAF), National Eligibility Criteria, Department of Health,
National Framework for NHS Continuing Care

, The Common Assessment Framework is a process used to identify and address unmet needs
in children. The Common Assessment Framework is used by an assessor to work with the
child and their parents, guardians and carers to understand the problem and create an
action plan to provide the best possible care for them. If a child is not receiving the best
results, such as having fallen behind in school, the Common Assessment Framework can be
used to highlight issues and then find a solution. According to the NHS, these frameworks
are key to integrating health and social care services and should be used in order to lead to
reduced inequalities in a service provision, greater clarity, transparency and consistency and
also to have a greater emphasis on the individual person rather than the services. The
Department of Health is the UK government department that is responsible for the health
and adult social care policy in England. Their primary purpose is to improve healthcare
outcomes by improving the way that it is delivered. The purpose of the national framework
is to establish clear rules and methods for evaluating eligibility for the NHS continuing
healthcare. Finally, these frameworks are reviewed for assessment and eligibility and aim to
improve and give the best care possible for the individual. Frameworks can be beneficial for
both of my case studies because by addressing these needs can help increase and have a
huge positive impact on their overall health and well-being.
The Education, Health and Care Plan (EHCP)
An EHCP plan is for young individuals up to the age of 25. It is for those who require more
support for their special educational needs than what is already available and there for
them. This plan can be requested by a parent, teacher, a health visitor, a GP or a nursery
worker. The local authority then carries out an assessment if a child is thought to need an
EHCP plan. This would have benefitted Tremayne when he was younger as he may have
required more support due to his autism.
P7- Explain how multi agency and multidisciplinary teams work together to provide co
ordinated support.
Multi-disciplinary Teams
A multi-disciplinary team is a team formed of professionals in the same service who have
different roles. Professionals can use their skills together to ensure that the individuals get
better care and services from the provider. When working together, there may be potential
conflicts that may happen. When dealing with these conflicts professionals must ensure that
the individuals’ best interests are involved and are their main priority. This can be beneficial
by showing Tremayne the final results he could potentially achieve if he collaborates
affectively with the professionals.
Health Care Professionals
A range of health care professionals are used within a multi-disciplinary team, depending on
the patients’ personal needs.
