Discovery Phase
In the past year, there have been several specific examples of how our care
setting has exemplified our focus on quality and safety. An example was the successful
handling of a critical incident by the Rapid Response Team (RRT) in the oncology unit
where a patient had a cardiac arrest. Due to the RRT’s timely response, organization,
and adherence to the ALS guidelines, the patient was successfully resuscitated and
stabilized. In triangulating family feedback and data on response times and the success
of interventions, this event showcased our teams’ communication, mastery of modern
equipment, and commitment to safety. Additional support for themes of teamwork,
safety protocol and technological competence was provided by our hospital’s Joint
Commission accreditation as well as its recent award for Emergency Department
excellence.
Another great achievement was the overall decrease in Hospital Acquired
Infections (HAIs) by employing strict hand washing procedures and policies, staff
education and awareness, and strict infection control measures. These practices
reduced HAIs by an impressive 40% that was evidenced by monthly audit reports, staff
training records and commendation from the local health department for exemplary
performance in infection control. It has further cemented our steadfast dedication to
patient safety, staff education, and adherence to best practices, as evidenced by our
accreditation with the National Quality Forum (NQF) and better patient satisfaction
scores.
