Definition of Nursing Process : To identify, diagnose, and treat human responses to actual or potential health problems to health and illness. It is a systematic method that directs the nurse and patient , as they together accomplish the following 5 steps: Assesing, Diagnosing, Planning, Implementing, and Evaluating ( should be individualized and organized) Assessing : Systematically collecting and verifying patient data. Diagnosing : Clearly identifying patient strengths and actual and potential health problems that ursing intervention can resolve. Planning (Outcome Identification) 2 : Develop a holistic plan of individualized care that specifies the desired patient goals and related outcomes and the nursing intervention most likely to assisst the patient to meet those expected outcomes Implementing : Execute the plan of care Evaluating : Evaluate the efectiveness of the plan of care in terms of patient goal achievement. Identify positive or negative outcome achievement. Revise if necessary.
