PN Comprehensive Online Practice 2023 Coordinated Care (2 items) • Establishing Priorities (1 item) Prioritizing Client Care (Active Learning Template: Basic Concept) There should always be questions to ask yourself when prioritizing. 1. Is this an acute problem or a chronic problem? 2. What can I fix the most quickly? 3. What could happen to the patient if I fail to act on this right now? (Think about the patient’s safety and any life-threatening situations.) 4. What can I delegate to maximize results? 5. What can I delay safely so that I can get something more important done now? (Sometimes you must delay routine things when the unexpected comes up) 6. What can I do as the nurse and what do I need to get an order for? 7. Which treatment, diagnostic, assessment will make the most impact for the patient’s condition and should take priority? • Legal Responsibilities (1 item) Reporting Client Conditions to a Regulatory Agency (Active Learning Template: Basic Concept) Nurses are legally mandated to report abuse, neglect, gunshot wounds, dog bites, some communicable diseases and any unsafe and/or illegal practices done by another health care provider. Some examples of these practices and behaviors include improper client care, narcotic diversion, unsafe staffing practices and substance abuse by a member of the healthcare team. Health Promotion and Maintenance (1 item) • Developmental Stages and Transitions(1 item) Reinforcing Teaching About Puberty with Female Clients(Active Learning Template: Growth and Development) Females usually stop growing about 2 years after menarche. The final 20% to 25% of height is achieved during puberty. Acne may appear during adolescence. Girlsstop growing at about 2 to 2.5 years after the onset of menarche. They grow 5 to 20 cm (2 to 8 in) and gain 7 to 25 kg (15.5 to 55 lb.). Psychosocial Integrity (1 item) • End-of-Life Concepts (1 item) Caring for an Actively Dying Client (Active Learning Template: Basic Concept) The role of the nurse during the active dying phase is to support the patient and family by educating them on what they might expect to happen during this time, addressing their questions and concerns honestly, being an active listener, and providing emotional support and guidance. Basic Care and Comfort (2 items) • Elimination (1 item) Expected Findings of DiabetesInsipidus(Active Learning Template: System Disorder) Extreme thirst that can't be quenched (polydipsia) Excessive amounts of urine (polyuria) Colorless urine instead of pale yellow. • Nutrition and Oral Hydration (1 item) Caring for a Client Who Has a Gastrostomy Tube (Active Learning Template: Nursing Skill) Flush your child’s G-tube with water, before and after any tube feeding, before and after any medications and at least every eight hours. Remove old dressing (if dressing is being used). Look at the area where the tube enters the skin. Check for redness, swelling, any drainage or excess skin growing around the tube. A small amount of clear tan drainage can be normal. Wash skin around the tube with soap and warm water. Clean around G-tube to remove any drainage and/or crusting. Rinse soap off with clear water. Dry skin thoroughly. Keep this site clean and dry. A dressing may be used around the G-tube site as instructed by your care team. Do not use lotions or ointments around tube site unless directed by your care team. Reduction of Risk Potential (1 item) • Diagnostic Tests(1 item) Reinforcing Teaching About Lumbar Puncture (Active Learning Template: Diagnostic Procedure) Instruct the patient to empty the bladder and bowel before the procedure. Do vital signs monitoring and neurologic assessment of the legs by assessing the patient’s movement, strength, and sensation. Place the client in a lateral decubitus position. Assist the client to assume a lateral decubitus (fetal) position, near the side of the bed with the neck, hips, and knees drawn up to the chest. An alternative position is to have the patient sit on the edge of the bed while leaning over a bedside table. Instruct to remain still. Explain that he or she must lie very still throughout the procedure. Any unnecessary movement may cause traumatic injury. Physiological Adaptation (2 items) • Alterations in Body Systems (2 items) Caring for a Minor Burn (Active Learning Template: Nursing Skill) The area of tissue damage should be cooled with cool running water for 20 minutes. Cooling for longer than 20minutes is not beneficial. Ensure the unburnt areas of the patient are covered and warm to prevent hypothermia. • Teaching About Preeclampsia (Active Learning Template: System Disorder) Say high blood pressure rather than hypertension to assure a complete understanding and severity. Be sure patient knows the symptoms and exactly what to do if she experiences any of them. severe headaches. vision problems, such as blurring or seeing flashing lights. severe heartburn. pain just below the ribs. nausea or vomiting. excessive weight gain caused by fluid retention. feeling very unwell. sudden increase in oedema – swelling of the feet, ankles, face, and hands. Assess patients understanding. Try asking questionssuch as "Have you experienced any changes in your eyesight? Have you had any really bad headaches that won't go away, even with medication?" Ask her what she would do if she did experience any of those symptoms. Is she supposed to call the nurse's line or head directly to labor and delivery? Make sure patient is fully aware.
