I concur with your statement of KB meeting the criteria for post-traumatic stress disorder
(PTSD). According to Friedman (2018), in order to diagnose PTSD, patients must show at least 1
intrusion symptom, 1 avoidance symptom, 2 symptoms from the negative cognitions and mood
group, and 2 symptoms from the arousal and reactivity group. Lastly, patients must have
continual symptoms for at least one month after experiencing the traumatic event along with pain
and suffering or impairment in functional activity (Friedman, 2018). It has also been proven that
patients with PTSD report a higher degree of substance/alcohol abuse, somatic symptoms, and
pain (Greene, Neria, & Gross, 2016). Due to these complaints, patients usually visit their primary
care provider initially, therefore increasing the use of healthcare services (Greene et al., 2016).
According to Greene et al. (2016), they concluded that there is a high rate of patients undetected
with PTSD in the primary care setting ranging from 0% to 52.1%. Possible reasons for the poor
rates include: patients may not pursue to be diagnosed with PTSD due to them not understanding
the association between trauma exposure and their own symptoms; or because of stigma, which
is highly applicable to military veterans (Greene et al., 2016). Additionally, primary care
providers may not fully understand, or do not feel confident enough with PTSD and its
associated symptoms resulting in a missed diagnosis; or because of a disconnect between
primary care and mental health services (Greene et al., 2016). According to Kearney and
Simpson (2015), the Primary Care PTSD screening (PC-PTSD) and the PTSD Checklist (PCL)
performed the best in a study involving 15 different screening tools for PTSD, which may make
them practical for use in the primary care setting. The PCL is a 17-item screening tool (Kearney
& Simpson, 2015).
