This report will be based on Fatima who is a 49-year-old mother who lives with her 2 children and husband. She has
been currently unemployed and spends a lot of time at home. Fatima has been diagnosed with type 2 diabetes and
complains of tiredness, daytime sleepiness, frequently waking up at night to urinate and is significantly overweight.
There is also a family history of diabetes her mother and sister both suffer with type 2 diabetes. This report will provide
a justified and personalised treatment plan that reflects the needs, wants and wishes of Fatima whilst ensuring that his
physical, emotional social and intellectual needs are met ensuring a holistic approach to his healthcare.

condition will worsen and would stop her fulfilling her religious beliefs and practices.
