Rights of Medication Administrations Right client Right medication Right route Right time Right dose Right documentation Right Client – two forms of identifiers (some facilities use barcode scanners) Right medication- read prescription carefully, question medication if it doesn’t seem right for the patient, use three checks (once before removing it from shelve, once after taking it out of container, and lastly at the bedside). Right Route – compare the label on MARS and then on label of medication, clarify with provider if confused. Right Time – follow facility guideline. With medications needed to be given every 4 hours, 6 hours etc give within 30 minutes of next dose time. Right Dose – match prescriptions, perform conversion, and calculate dosages. (Have another nurse check your math if it’s a large or smaller dose than normal). Right Documentation – immediately after administration. Follow facilities protocol if medication isn’t given on time.
