D4 - Evaluate how multi-agency and multidisciplinary working can meet the care
and support needs of specific individuals.
Aisha
In the case of Aisha H. the multi-agency team that involved the local authority, CCG
( clinical commission group) and the Alzheimer's Society were able to effectively support
her because they were able to meet her needs based on her having dementia, which she
can’t do anything about. Therefore, the multi agency team has used the right services
such as successfully able to identify what type of resources or activity will be required
for her, as for her communication the team has been able to provide different
specialists that are qualified in helping those who communicate is a barrier. In this case
they were able to support Aisha communication issues by adapting the right skills
needed to help her communicate with the professionals. The team has also been able to
maintain the Care Act 2014 that is a legislation that helps improve people’s
independence and wellbeing, by introducing Aisha to different communication methods
and facilities that were made for individuals that has dementia as Aisha who have
ongoing care and support, which have be managed with specialists and medication that
can contribute on helping Aisha with her dementia as it going further and improving her
day-to-day lifestyle. Although in the other hand the multidisciplinary team work has
worked the way it was supposed to, as the geriatrician and mental health specialist
weren’t able to fully meet her needs due to different circumstances such as being
incapable to assess her needs in the right way as they were all focused on helping
individually instead of sharing and teaming up to come up with the right approach. In
order to overcome this my suggestion would be coming up with differences and
similarities of their role as professionals in order to provide a good healthcare service
that involves good teamwork and communication skills needed to support her. As most
of these professionals are mental health specialists the fastest and most effective way
they could help Tremayne is by also sharing their knowledge and assessing her progress
daily as this will allow them to know if their job is helping her or not and manage to
come up with other solutions.

As we seen previously in this assessment the multidisciplinary team and multi agency
team has worked together in order to meet Valerie needs, by also facing some positive
and negative outcomes from it, in the case of the multidisciplinary team that included
