NSG C304 - Portfolio Rough Draft. Questions and Answers. Complete Solutions. A. Complete all your documents using the Web Folio Builder (in Taskstream) that you used to create your portfolio in the Professional Roles and Values course. 1. Professional mission statement I want to give excellent patient care and learn to treat my patients with a wholistic approach, seeing the person that is my patient. My career goals are to empower patients to take ownership of their care and understand their disease process. I would like families to embrace the understanding of health and understand when treatment options no longer benefit their loved ones. I want to meet patients where they are to provide optimal quality of life. I am pursuing my MSN to stay sharp and abreast of new developments in care that can benefit my patients and their families to forge trust in both their community and the medical community. I want to focus my energies in the areas of oncology, palliation and hospice. Oncology has taken on a new face and it has become a common disease that has reshaped our view of cancer and new life saving treatments are changing every day. To allow symptom management during the treatment process and after life- saving treatments have failed is an integral part of offering patients and families quality time that they might not otherwise cherish. I want to educate on how to live with the disease and accept the transitions of life. I want to help create interdisciplinary doctrine to help promote collaboration between teams. a. Reflect on how your professional mission statement will help guide you throughout your nursing career. I started my nursing career as a Medical Oncology nurse on a floor with a telemetry sub unit. Many of my patients were unaware of new developments in their disease progression and over the course of a year or 2 they repeatedly returned with uncontrolled pain and nausea. Majority of these patients had only seen palliative care at the start of the pain and not had seen anyone from hospice until they were in the active phase of dying. I became a home hospice nurse to focus solely on end of life care and learn how to better care for my patients on their terms. The organization I am employed by owns a partial stake in the hospice company I moved to, I am hoping to learn all angles and complete research to help bridge this gap and promote patient understanding and quality of life prior to peaceful transition. I will use this to keep communication and collaboration at the forefront of all my learning, medical and ethical decisions throughout my nursing career. These two provisions influence my professional nursing practice by encouraging me to adopt a patient-centric approach to the management of patients. Under Provision 1 I will protect the patient’s privacy and not treat them like a specimen. Closing the door or curtain and keeping their body covered when others are present. I will encourage the patient to take an active role in their ow care and take time to listen to their concerns and explain anything they do not understand. As a nurse, I will respect and honor their customs and individualize my care to fit my patient under the guidelines set forth before me. Under Provision 2 I will include the family and personal care givers per the patient’s request and realize that my patients care affects the entire family unit. I will protect my patient and myself from infection by observing proper cleanliness and respect between multiple rooms and common areas. I will extend my respect to members of my family and community to do my part in promoting a healthy community beyond the walls of my health care facility. 2. Complete a professional summary that includes the following: a. Explain how the specific artifacts or completed work or both in your portfolio represent you as a learner and a healthcare professional. In my first semester, course C820 (Professional Leadership and Communication), I learned to be aware of my leadership style and my nursing role. I learned how to connect and collaborate with our leadership styles and later how to be a part of a team that allows my strengths to be incorporated to better serve my patients. In my second semester I learned statistics and research and how to do evidence-based practice
