The quality of patient care and patient safety is a major priority in any healthcare settings. Irrespective of the healthcare environment it is the responsibility of the nurse to provide a culturally competent holistic and patient focused care. To limit adverse patient events, reduce medical errors and reduce additional costs to the facility it is imperative to perform a needs assessment of the organization. This will significantly impact patient safety and care especially before, during and after any nursing and medical interventions. In this Capstone Change Project, the preceptor and the student met and performed need assessment of the organization and have chosen to research on one of the following topics: 1. Role of the nurse in preventing inappropriate indwelling urinary catheter and Catheter Associated Urinary Tract (CAUTI) prevention in the home caresetting 2. Role of nurse in reducing the incidences of fall in home care. 3. Implications of high nurse turnover and inadequate staffing on patients’outcome.
