Meaningful changes must always be based upon evidence. We can look to the words of Florence Nightingale for the foundation of evidence based practice when she stated “what you want are facts, not opinions” (Nightingale 1895, quoted in American Nurses Association, 2010, p.15). Not only do nurses have to look at the facts (data), but the data has to be examined critically to evaluate how it will apply to the particular situation being questioned. The John Hopkins Nursing Evidence-based practice model (Dearholt & Dang, 2012) provides a good structure for this task. I look forward to your critical evaluation of your workplaces to determine the direction of your project and the changes that you will be working on making to increase the quality of patient care! ReplyShow Less Lesline Joseph Lesline Joseph 10/23/2016 11:35:06 AM Establishing Evidence-Based Practice in a Nursing Setting Hello Professor and Class, Implementing EBP is a priority in the healthcare profession and nurses should have the knowledge and skills in implementing it in whichever practice settings they are in. In my previous nursing setting, caring for the patient at bedside on a daily basis is one way we utilize research through EBP to ensure positive patient outcome. Some of the challenges faced were shortage of nurses and work overload, inadequate access to the internet, reluctance by seasoned nurses to changes, language barriers, patients unwillingness to change their routine, lack of incentives and low priority by management on research and educational training. In order to effectively put evidence into practice communication and working together as a team, with everyone working toward the same goal is important, management should ensure that the nursing units are adequately staffed or hire more nurses with advanced degrees, provide the necessary resources such as, access to computers on nursing units, offer incentives (tuition reimbursement) for continuing education, provide interpreters for patients with language barriers and offer educational information to patients who are resistant to changes. The practice of nursing is founded on nursing knowledge, and nursing knowledge is generated and disseminated through reading, using, and creating nursing research. Evidence-based practice is important in nursing because outcomes is improved, care is more efficient and effective, and errors are reduced when nurses use evidence as a standard of care. If we can look at each patient encounter with an open mind we can evaluate what is working, what is strained, and what we need to re-evaluate. According to our CCN week 1 (2016) lesson, "Whenever an issue seems to be a problem, one of the first steps in addressing the issue is assessment of the presence, extent, and seriousness of the issue" (Baseline Assessment of a Problem, para 1). Keeping this evaluation tool in mind, each concern we come across can be assessed right then and there. As discussed by Finout-Overholt, Baldwin, and Greenberg (n.d.), "In Step 1 of the EBP process, asking the searchable, answerable question, clinicians must consider using a standardized format" (para 3). We do not have an EBP committee, but I believe that our Director and Assistant Director will be open to whatever suggestions and recommendations we offer. For example patient education, EBP would necessitate that we educate patients in a manner that is most effective. This education would utilize practices that are new to nursing and would be more effective, practices like teach back and stoplights. These are new to patient education but may lead to a better understanding of patients concerning their illness and not necessarily take more time or nurses to accomplish. An article in the Journal of Clinical Nursing illustrates how ineffective our patient education is currently. As for self-care confidence, participants were less confident in regard to keeping themselves free from HF symptoms and doing something to relieve their symptoms. This may indicate that the patient sample in the present study had low levels of self-care management. Self-care confidence is the level to which patients think about their ability to perform self-care decision-making. (Tsai, Waaang, Lee, Tsai, & Chen, 2014) Necessity is the mother of invention. When resources are limited nurses are inspired to discover how to do a better job with the resources that are available. EBP hopefully is the tool we look to.
