VITAL SIGNS MONITORING
Blood Pressure (BP):

FLUID BALANCE CHARTING (INPUT/OUTPUT
MONITORING)
Input Monitoring:
Accurate documentation of all fluid intake, including oral fluids,
intravenous infusions, and medications, is essential for effective heart
failure management. Careful fluid tracking helps prevent fluid
overload, which can worsen symptoms such as shortness of breath and
edema, leading to further complications. Proper fluid management
