Pressure injuries remain an ongoing significant concern in long-term care settings due to
the high prevalence of immobility, chronic illness, and age-related skin fragility among residents
(Sugathapala et al., 2023). These injuries can lead to pain, infection, delayed healing, and
decreased quality of life making prevention a priority in nursing care (Asiri, 2023). Although
pressure injuries are largely preventable, they continue to occur due to inconsistent prevention
strategies and gaps in clinical practice (Sim et al., 2024).
Nurses play a critical role in identifying at-risk patients and implementing evidence-
based interventions to prevent skin breakdown. These interventions include turning and
repositioning, routine skin assessments, and moisture management (Kandula et al., 2025). The
purpose of this paper is to present a capstone proposal focused on reducing pressure injuries in
long-term care by providing a structured prevention plan supported by evidence-based
interventions, staff education, improved documentation practices, and use of healthcare
technology. This paper will discuss the project proposal, background, literature review, methods
of searching, findings, objectives, strategies, evaluation, and budget.
