She denies fever, weight loss, or recent illness. No new medications. However, her mother notes
that the patient recently started using a new scented body wash and detergent. The patient is
active in school and recently joined a swim team.

She has a past medical history of mild seasonal allergies. No history of asthma or eczema
diagnosed previously. No known drug allergies. Immunizations are up to date.

Family history is notable for asthma in the mother and allergic rhinitis in a sibling.

Chief Complaint (CC):
“Itching all over my body for 3 weeks.”

History of Present Illness (HPI):
13-year-old female with a 3-week history of progressively worsening generalized pruritus.
Symptoms are worse at night and after bathing. Reports excoriations due to scratching. Denies
rash at onset but now has visible skin changes. No associated fever, fatigue, or systemic
symptoms. Possible triggers include new scented soap and laundry detergent. Swimming
exposure noted.

Past Medical History (PMH):

• Seasonal allergic rhinitis
• No chronic dermatologic conditions
