initiative is important because sepsis triggers a severe body-wide
inflammatory response to infection that can be fatal. By identifying those
with sepsis and treating them effectively, we reduce mortality from this
condition.
Initiative 2: Reducing Avoidable Readmissions.

This initiative focuses on improving the discharge process to ensure safe
transitions back to a patient's home or to a skilled nursing facility. A key
component in this effort involves identifying patients who are at risk for
readmissions; these include patients with conditions such as congestive
heart failure and pneumonia, and taking extra steps to support their
discharge. This program empowers patients and their families by providing
comprehensive education about their medications and potential side effects.
This initiative is important because returning to the hospital can place an
enormous physical, emotional, and financial strain on individuals. It
addresses the massive systemic issue: national data indicate that almost one
in five Medicare patients are readmitted 30 days after discharge.
Initiative 3: Central Line-Associated Blood Stream Infections
(CLABSI).

To combat infections related to central line catheters, an ICU team at
Fielder Medical Center was asked to help eliminate catheter-related
bloodstream infections. The ICU team researched and developed two
simple, cost-effective best-practice checklists: one for catheter insertion and
another for line maintenance. This new program was named “Stop the Line,"
which retrained staff on consistent care techniques. This also empowered
staff to speak up and intervene if they suspected the sterile field had been
compromised. Central lines are life-saving tools used for delivering nutrition
and medication; they are historically prime targets for bacteria that cause
dangerous bloodstream infections. This initiative is necessary to protect
patients from avoidable pain, severe distress, and death.
