In this scenario we had Wilfred Price, 78-year-old male with history of dementia. Wilfred

presented to the ED via EMS after he was found wandering. Wilfred presented as agitated with

confusion and occasional combativeness. Wilfred’s assessment was normal, except for

tachycardia. His medical record confirmed history of HTN with daily medication regiment.

Wilfred lived with his daughter Janice who was contacted and notified about Wilfred’s status.

The scenario focused on managing agitated patient and providing patient’s safety with

appropriate nursing skills including focused assessment and communication.



Background

World Health Organization defined dementia as “a syndrome that can be caused by a number of

diseases which over time destroy nerve cells and damage the brain, typically leading to

deterioration in cognitive function effecting ability to process thought beyond what might be

expected from the usual consequences of biological ageing; while consciousness is not affected,

the impairment in cognitive function is commonly accompanied, and occasionally preceded, by

changes in mood, emotional control, behavior, or motivation” (World Health Organization, 2025).

Kennedy and colleagues pointed out that “behavioral and psychiatric symptoms in dementias are

more common in individuals with more advanced dementia and are associated with decline in

functional abilities, increased caregiver burden, and risk for abuse and neglect” (Kennedy et al,

2020).

Managing behavioral and psychological symptoms of dementia is a crucial aspect in nursing

practice. Understanding the condition and recognizing potential triggers of agitation can provide
