Evaluating the Effectiveness of Telehealth in Managing Hypertension among Adults

Hypertension remains among the most common chronic conditions globally and is

considered a major risk factor for cardiovascular diseases, stroke, and kidney failure (Mills et al.,

2020). Despite such advances in treatment and wide awareness campaigns, poor blood pressure

control has remained a feature among many adults due to barriers to care, which include lack of

access, poor follow-up, and nonadherence to prescribed regimens. The incorporation of

telehealth into hypertension management has been recognized as an innovative strategy that

leverages technology to promote better patient monitoring, improve patient-provider

communication, and enhance treatment adherence (Choi et al., 2020). Thus, through access to

patient progress via remote monitoring devices, virtual consultations, and digital education

platforms, nurses and clinicians are able to intervene on time and prevent complications. As

healthcare delivery increasingly shifts toward being technology-driven, assessing the

effectiveness of telehealth in improving outcomes in hypertension is appropriate for evidence-

based nursing practice.

The aim of this paper is to discuss how telehealth interventions contribute to the better

management of blood pressure in hypertensive adults and base decisions on evidence-based

practice. This includes discussion of the problem, strategies adopted in searching for credible

evidence, the level of evidence from the selected studies, and synthesis of findings from recent

literature. Further, it intends to outline practical nursing solutions that could be used to engage

patients more effectively and attain better long-term results through telehealth follow-up.

Identification of best practices based on quantitative and qualitative research will enable nurses

to consider more efficient models of care, which can bridge the gap between clinical visits and

home-based management. The overall purpose of this paper is to establish evidence-based

, understanding regarding the contribution of telehealth toward hypertension control and its

potential to remodel chronic disease management on grounds of accessibility, patient

empowerment, and continuous monitoring (Ezeamii et al. 2024; Abdullah et al. 2024).

Problem

Hypertension or high blood pressure remains a persistent global health problem and is

considered to be one of the major conditions which cause cardiovascular disease, stroke, and

premature death. Despite being highly preventable and manageable, the prevalence of

hypertension keeps on increasing and has affected almost one-third of the adult population

worldwide (Mills et al., 2020). The inability to adequately control blood pressure remains a

major concern even among patients who have already been diagnosed and treated. This results

primarily from the limited access to regular follow-up care, poor adherence to medications, and

lack of education regarding lifestyle modification for the patients. In the traditional model, office

visits create logistic and financial barriers to care, especially in patients living in areas that are

rural or resource-poor. For this reason, many patients are not adequately being managed with

blood pressure control, which ultimately contributes to increased morbidity, hospitalizations, and

costs of health care. To meet this ongoing challenge, health systems are embracing telehealth as

an innovative approach for improving the management of hypertension through remote

monitoring, engaging patients, and supporting them continuously with professional support.

Various reasons exist why poor control of hypertension management is still a burden. Choi

et al. (2020) view that poor control is majorly contributed to by patient nonadherence to

medication and lifestyle recommendations, besides being compounded by inadequate health

literacy, forgetfulness, or a lack of timely follow-through by the providers. Besides, socio-

economic disparities and geographical barriers make access to care poor, with a lack of regular
