NRNP/PRAC 6635 Comprehensive Psychiatric Evaluation
Mental illness affects 1 in 5 individuals in the United States, and mood disorders account
for 20.9 million of the population (Vieira, 2022). Mood disorders are mood fluctuations affecting
a person's emotional state. Being able to differentiate between the different mood diagnoses is
essential for the PMHNP to safely and effectively treat their patients. The purpose of this paper is
to document a comprehensive psychiatric evaluation of the patient in Video 18, providing
primary and differential diagnoses, and reflecting on what I would do differently next time in the
interview.
Subjective:
CC (chief complaint): “Everyone keeps saying I have PTSD. Even Luke says I have it.”
HPI: RC is a 25-year-old African American female serving the army presenting to the clinic for a
psychiatric evaluation for possible PTSD and reports a history of depression. RC currently lives
off base with her boyfriend. RC says she was treated with an antidepressant for depression when
she was 13 or 14 but quit taking her medication when she enlisted in the army. She is currently
not prescribed any medication. RC states recently she has been struggling to sleep, “tossing and
turning,” increased negative thoughts, “ I’m not worth it. Like I’m not good enough,” and
inability to concentrate. Reports she has been feeling this depression since she was 18 years old
and is worse when she is on her period. RC states she doesn’t want to go out or interact with
other people. She reports no sex drive unless initiated by a live-in boyfriend.
