Avery Brook is a 10-year-old female brought to the clinic by her mother due to
worsening academic performance over the past year. Her mother reports that
Avery was previously an above-average student but has recently experienced
declining grades, incomplete assignments, poor concentration, and increasing
forgetfulness. Teachers have noted that Avery appears distracted during class,
frequently loses materials, and has difficulty following multi-step instructions.
Symptoms occur both at school and at home. Avery often starts tasks but fails to
complete them and is easily distracted by external stimuli. Her mother reports
frequent careless mistakes in homework and difficulty organizing activities.
The symptoms have gradually worsened over the last 12 months and are beginning
to affect social relationships and self-esteem. No recent trauma, major life
