AACN AGACNP Review
Addison's sickness - ANS-Primary
Caused via harm to the adrenal cortex (autoimmune, TB, metastatic disorder, deposition
diseases, and drug brought about) leading to a lower in cortisol production.
Secondary
Caused through pituitary failure to release ACTH (in any hypopituitary ailment) causing a lower
in cortisol production.
Sudden withdrawal of systemic corticosteroids leading to a decrease in cortisol manufacturing
from precipitated corticosteroid suppression.
Aortic regurgitation - ANS-Grade 1-three/four high pitched blowing diastolic murmur heard
satisfactory at 3rd LICS.
May be better by pressured expiration, leaning forward.
Usually S3, extensive pulse strain, sustained thrusting apical impulses.
More commonplace in men commonly from rheumatic heart ailment and from time to time due
to tertiary Syphilis.
Aortic sclerosis - ANS-Grade 2-3/6 systolic ejection murmur first-rate heard at RICS.
Carotid upstroke full, no longer delayed, no S4, absence of signs.
Benign thickening of the aortic valve leaflets.
No exchange in valve pressure gradient.
50 over 50 (found in 50% of these over 50)
Aortic Stenosis Murmur - ANS-Grade 1-4/6 harsh systolic murmur, typically
crescendo-decrescendo sample.
Heard best at 2nd RICS, apex, and softens with standing.
Radiates to carotids, may have diminished S2, sluggish filling carotid pulse, slim pulse stress,
loud S4, heaving PMI.
Great the diploma of stenosis the later the height of murmur.
Younger adults because of bicuspid valve.
Older adults due to calcific /rheumatic.
Dizziness and syncope are an ominous sign pointing to severely decreased cardiac output.
Atrial Septal Defect ASD - ANS-Grade 1-3/6 systolic ejection murmur within the pulmonic
region.
Widely cut up S2, right ventricular heave.
Typically without signs and symptoms and then center elderly HF.
Persistent ostium secundium in mid septum.
Will solve with ASD correction.
, Bacterial Meningitis - ANS-Glucose <45 mg/dl
Protein >50 mg/dl
Opening strain is markedly high.
Cell count number 200-20,000 PMN/microliter.
Bouchard's nodes - ANS-Middle Joint of finger.
Brudzinski's Sign - ANS-Severe neck stiffness reasons a patient's hips and knees to flex when
the neck is flexed.
Cushing's Syndrome/Disease - ANS-cortisol extra typically because of pituitary adenoma
60-70% of instances.
Central weight problems w/ extremity losing.
Dorsocervical fats pad.
Rounded facies.
Spontaneous bruising.
Pink striae
hyperpigmentation
negative wound recuperation/ skin infections.
Dexamethasone suppression test.
1mg dexamethasone at 2300 hours and degree serum cortisol at 0800.
Remove sources of excess and manage effects ( HTN, hypokalemia, hyperglycemia.)
diabetes insipidus (DI) - ANS-Insufficient ADH or decreased sensitivity to ADH
Nephron can't conserve water.
Commonly resulting from damage to the pituitary gland or hypothalamus (surgical operation,
tumor, meningitis, head damage).
Can be nephrogenic where the kidney in unable to reply to ADH.
Serum: Hypernatremia and hyperosmolarity.
Urine: Hyponatremia and hypoosmolality.
Replaced ADH and supportive fluid replacement.
Drop Arm Test - ANS-Rotator cuff evaluation
Epidural Hematoma - ANS-Temporal or parietal skull fracture
