2. History of Present Illness
Grady is an 18-month-old male brought to the emergency department by his mother due
to a two-day history of worsening cough and respiratory symptoms. Per his mother, the
illness began approximately 48 hours ago with clear rhinorrhea, congestion, and a
low-grade fever of 100.4°F (38.0°C) measured rectally at home. His appetite was slightly
decreased, but he was still taking fluids well and had a normal activity level. She treated
the fever with a single dose of acetaminophen, which was effective.
Approximately 12 hours prior to arrival, around 2:00 AM, the mother was awakened by
Grady's cough. She describes the cough as "sudden," "harsh," and sounding "like a seal
barking." She had never heard him cough like this before. With the cough, she noticed a
