History of Present Illness (HPI)
Mrs. Johnson is a 54-year-old female presenting with acute onset lower right abdominal pain that
started approximately 8 hours ago. She describes the pain as sharp, constant, and 8/10 in intensity,
radiating slightly to her back. She reports nausea, two episodes of vomiting, and loss of appetite.
She denies diarrhea but notes mild bloating. The pain worsens with movement and coughing. She
reports no previous episodes like this.
Additional details:
No recent trauma or heavy lifting.
No urinary symptoms except mild dysuria.
Denies fever at home but feels flushed.
She reports mild fatigue and generalized malaise.
Past Medical History (PMH)
Hypertension, diagnosed 8 years ago – currently on amlodipine 5 mg daily.
Hyperlipidemia – on atorvastatin 20 mg daily.
No history of abdominal surgeries.
No previous GI conditions.
