18-Year-Old Male Patient
Reason for Encounter: Syncope (Passed Out)
1. Chief Complaint (CC)
“I passed out earlier today while standing in line at
school.”
2. History of Present Illness (HPI)
, The patient is an 18-year-old male who presents after
experiencing a syncopal episode earlier today. The
patient reports that he was standing in line at school
when he suddenly began to feel lightheaded, dizzy,
and warm. He noticed tunnel vision and ringing in his
ears before losing consciousness. Witnesses report
that the patient collapsed to the ground and was
unconscious for approximately 30 seconds.
The patient denies experiencing chest pain,
palpitations, shortness of breath, or seizure-like
activity prior to or after the episode. There was no
tongue biting, bowel or bladder incontinence, or
postictal confusion. He regained consciousness
