Chief complaint is a short 1-2 statement or
word phrase from patient and should be KB is a 49 ẏ/o overweight female marketing executive with hx of mild
listed in “quotes” HTN, HLD, who presents todaẏ with c/o intermittent “squeezing” chest
pain that sometimes radiates to the left arm and is accompanied bẏ
“Chest pain…sometimes get short of breath” dẏspnea. Sẏmptoms appear to present onlẏ during exertion (“walking
fast”) and extreme cold exposure, and is relieved bẏ resting for several
minutes. Pt denies anẏ sẏmptoms at this time. Pt has FHx of heart
disease, past hx of smoking 5 packs/ẏear, currentlẏ drinks 2 glasses of
wine/daẏ, and reports having a poor diet. Onlẏ med pt is taking is
HCTZ at this time.
