Phase 1: CPE ACTIVITY EVIDENCE & TASK DELIVERABLES

Include the following deliverables in your e-portfolio:

1a. a paragraph that describes population health and the role that Healthy
People 2030 Social Determinants of Health play in guiding efforts toward
improving population health

Population health entails understanding and handling the different elements that
affect health outcomes of diverse populations. These elements include political
determinants of health (PDOH) that establish the environment that weakens
population health (Dawes & Gonzalez, 2023). Also, SDOH affect individual's
wellbeing and health and prompt the significant health inequalities and disparities.
Therefore, Health People 2030 SDOH assists in identifying the “upstream factors”
such as housing, education, income, and access to nutrition, which do not impact
healthcare delivery, but prompt health disparities and subsequently affecting
population health (Office of Disease Prevention and Health Promotion, n.d.).
