SECTION 1: PATIENT 1 (Group A) – Case Scenario
*65-year-old male, right-sided weakness, slurred speech. Time last known well: 2 hours ago.*
Q1. 1a. Level of Consciousness (LOC):
The patient opens his eyes only when physically stimulated (shake shoulder). He does not open
to voice.
• 0 = Alert
• 1 = Drowsy
• 2 = Obtunded
• 3 = Unresponsive
Q2. 1b. LOC Questions:
When asked his age and current month, he responds incorrectly to both questions.
• 0 = Answers both correctly
• 1 = Answers one correctly
• 2 = Answers neither correctly
