History of Present Illness (HPI)
The patient is a 42-year-old male who presents with acute onset diarrhea for 3 days. He
reports passing 6–8 episodes of loose, watery stools per day, without formed stool. The
diarrhea began suddenly and has progressively worsened. He describes the stool as watery,
brown, and foul-smelling, without visible blood or mucus.
Associated symptoms include:
Generalized weakness
Dizziness on standing
Dry mouth and excessive thirst
Mild diffuse abdominal cramping (rated 3/10), relieved after bowel movements
Decreased urine output over the past 24 hours
