Power Form Optimization Proposal
Currently, bedside nurses are manually entering Non-Intermountain Lab Results for each
patient into 16 different iCentra Power Forms along with the normal values for all the labs. The
recommended process for developing this system optimization change would be to add the
custom default value “See Scanned Report” to each reference range once a value is entered into
the patient's lab result. The purpose of implementing this system optimization, we hope to
increase the utilization of these Power Forms, decrease manual charting errors, and streamline
the process of EHR documentation.
The Institute for Healthcare Improvement (IHI) Triple Aim has three dimensions that help
to optimize the performance of the healthcare delivery system. They define these dimensions as,
“improving the patient experience of care, improving the health of populations, and reducing the
per capita cost of health care,” (The IHI triple aim: IHI 2023). The system optimization proposal
relates back to all three of the Triple Aim objectives. For the improvement of patient experience
of care by implementing this system optimization it means less time can be spent charting and
more time at the bedside. By implementation, the health of our populations can increase due to
the increased utilization of these Power Forms. The more communication and information we
have regarding the patient's status the better for integrative care. And by implementing this
system optimization the patient‟s cost will go down as the frequency of the forms use will go up,
while the orders for repeat and duplicate labs will go down.
The optimization of this system to monitor and report progress on one aspect of an IHI
Triple Aim dimension would be to optimize the 16 Non-Intermountain Power Forms and
measure the utilization of the updated Non-Intermountain Power Forms.

The scholarly theory that will act as the project‟s theoretical framework is the Change
Theory. “Change theory is the study of change in individuals or social systems such as
organizations. Understanding change theory provides a framework for effectively planning and
implementing change in social systems and organizations. Healthcare information systems have
a major impact on the structure and functions of healthcare delivery systems. They bring about
significant change. The approach to managing the change process may result in a more effective
and efficient healthcare delivery system or it may result in increased dissatisfaction and
disruption,” (Nelson & Staggers, 2014).
Changing the way documentation is done can come across as more burdensome for the
end user. “The most common change for health professionals and informaticians is the
introduction of or upgrade to a health information system. A commonly used model of the stages
within the change is the System Life Cycle (SLC),” (Nelson & Staggers, 2014). This proposed
system optimization aims to make the workflow easier, in turn increasing the number of times
these Power Forms are documented on.
Theoretical Framework and Process
The theoretical framework that underlies this proposal is the Change Theory and the
System Development Life Cycle. This will include four phases that will ultimately have the
system enter the default value “See Scanned Report”. This system optimization change will save
bedside nurses valuable time and allow them more time to focus on physical patient care. This
