Situation:
My name is Abygelle and I am the student nurse from George
Brown College in charged for BH today. BH is a newly admitted
client, and she has been showing significant anxiety about her
current health, upcoming surgery, and the possibility of going to
a long-term care home.




B
Background:
BH is a 77-year-old female client, admitted to respite care today
at around 1300 h. She will be staying here for 10 days while her
daughter, the primary caregiver, will be on vacation. BH has
severe osteoarthritis in the right hip and mild in the left hip. She
has been experiencing difficulty in performing ADLs due to pain,
and this has caused frustration as she is struggling with the loss
of independence. She takes Tylenol and Celecoxib twice daily for
pain and Percocet for severe pain. BH has an upcoming hip
replacement surgery in 3 months. She has showed anxiety of
falling, anxiety about her upcoming surgery and the possibility
of moving to a long-term care if surgery doesn’t go well. She has
also been resisting to use the cane because it makes her feel
“old and frail”.

Assessment:
I have just assessed BH, and it seems like she is struggling with
her emotional well-being. She is very frustrated about losing her
independence because of the pain, and anxious about the
possibility of moving to a long-term care home after her surgery.
Her fear of falling might also lead her to avoid in participating in
activities or engaging with others.




R
Recommendation:
My recommendation is to provide emotional support to the client
to ease her anxiety, especially her fear about the surgery and
moving to a long-term care. Monitor her for falls and encourage
her to use the cane for safety but also in a way that does not
damage her self-esteem. I also recommend contacting her
doctor and ask for an increase in her medication dose for pain so
that she will be more independent and ambulatory.
