Case Study #5 - Phil Client Concerns The client, Phil, presents as a 15-year-old male of Hispanic descent. The client is present today at the request of his mother. The client currently resides with his mother and three sisters. His mother and father separated two and a half years ago, finalized the divorce one year ago, and his father remarried approximately 4 months ago. Phil states that since his father’s remarriage he was been sad, but also angry. He had hoped his parents could reconcile but came to the realization after his father’s remarriage that occur. His mother is reporting bouts of verbal aggression recently within the home as well as with teachers and peers at school. She is requesting assessment and treatment options Symptoms Behaviors Stressors Sleep disturbances Verbal Aggression Parental separation Sadness/Hopelessness/Anger Verbal Outbursts Paternal remarriage Loss of interest Loss of interest in peer interaction Puberty Assessment My recommendation for assessment would be the Diagnostic Interview for Adjustment Disorder (DIAD). The DIAD is a 29-point interview that looks to identify stressor symptoms, as well as an evaluation of functional impairment and/or distress. While there has yet to be exhaustive research on the efficacy of this assessment, it does appear to return “moderate to good” indicative results. The Adjustment Disorder – New Model (ADNM) has been developed, however, it is geared toward an ICD-11 diagnosis as opposed to DSM-V criteria. It would also be highly recommended to engage in real-time monitoring for suicidal ideations or behavior, as the presenting symptoms can often be indicating pre-cursors to an attempt at suicide. (Kleiman & Nock, 2018)
