The Role of Social Media in Cholera Health Communication and Community Empowerment
in KwaDukuza, KwaZulu-Natal

1. Introduction

Cholera remains a persistent public health threat in sub-Saharan Africa, driven primarily by
inadequate water, sanitation, and hygiene (WASH) infrastructure, climate variability, and
socioeconomic inequalities. In South Africa, recurrent risks of cholera outbreaks have been
linked to failing wastewater treatment systems, contaminated surface water, and uneven
access to safe drinking water, particularly in provinces such as KwaZulu-Natal (KZN).
KwaDukuza Local Municipality, located within the iLembe District Municipality,
exemplifies these vulnerabilities. With a 2022 population of approximately 324,912 residents
(Statistics South Africa; municipalities.co.za), the area features a mix of urban nodes such as
Ballito and Stanger (KwaDukuza), peri-urban settlements, and rural communities where
access to reliable piped water and sanitation is incomplete. While formal dwelling rates and
electricity access are relatively high, flush toilet connections and consistent safe water supply
remain uneven, creating conditions conducive to waterborne disease transmission (municipal
demographic profiles; WaterCAN and related environmental monitoring reports).

Simultaneously, South Africa has experienced rapid growth in digital connectivity and social
media use. By late 2025, social media user identities numbered around 29.1 million
(approximately 44.9% of the total population and over 64% of adults), with WhatsApp,
Facebook, TikTok, and YouTube dominating engagement (DataReportal Digital 2026 South
Africa; MAPS data). Daily social media time is substantial, and platforms serve as primary
channels for information seeking, peer discussion, and community mobilisation, especially
among younger and urbanising populations. This digital landscape presents both
opportunities and risks for health communication: opportunities for rapid, low-cost
dissemination of accurate prevention messages, participatory dialogue, and empowerment;

, and risks of misinformation, digital exclusion of the poorest or less literate, and uneven trust
in official sources.

This research investigates the role of social media in cholera-related health communication
and community empowerment specifically within KwaDukuza. It examines how residents
access, interpret, share, and act upon cholera information via social media platforms; the
extent to which these platforms facilitate or hinder empowerment (understood as increased
knowledge, self-efficacy, collective action capacity, and influence over local health
responses); and the interplay between digital communication, traditional health systems, and
community structures. The study is timely given ongoing regional cholera pressures in
Southern Africa, documented water quality challenges in KZN rivers and household systems,
and the proven utility of social media and influencer strategies in other African cholera and
infectious disease responses (for example, peer communication analyses during the 2024
Lagos outbreak and South African COVID-19 vaccine campaigns). By focusing on a specific
South African municipality, the research generates contextually grounded insights that can
inform municipal, provincial, and national risk communication and community engagement
(RCCE) strategies.

The proposal proceeds through a detailed examination of context, problem formulation using
the 4W & 1H framework, research questions, rationale, objectives, literature review,
theoretical framework, paradigm, design, methods, timeframe, data collection, population
and sampling, analysis, ethics, and conclusion, followed by sources and addenda.

2. Context and Background of the Research

KwaDukuza Local Municipality forms part of the iLembe District on the KZN north coast.
Population growth has been rapid (from 231,125 in 2011 to 324,912 in 2022), with a
youthful age structure (approximately 24.7% under 15 and 68.9% aged 15–64) and a sex
ratio of about 93 males per 100 females. Racial composition includes a Black African
majority (around 71%), a significant Indian/Asian population (approximately 18–19%),
White residents (around 8–9%), and smaller Coloured and other groups. Language use is
dominated by isiZulu, with English also widely spoken. Household services have improved
