Please complete everything in this document and provide detailed answers. This document
summarises everything you have put into practice over the past five weeks at the school – you
have worked hard and want to be credited for it. The following marking grid is used to mark this
portfolio.
PLEASE TAKE NOTE THAT YOUR PORTFOLIO WILL NOT BE MARKED, AND YOU WILL
RECEIVE A 0 MARK IF YOU HAVE NOT COMPLETED THE FOLLOWING:
1. The student declaration form on page 4.
2. The declaration form with signatures, dates, and the school stamp on page 5.
3. The attendance register on pages 6-7.
4. Your mentor-teacher’s evaluation report.
5. Your teaching practice was completed in a Grade 4, 5, or 6 classrooms only.
6. An original portfolio that was written by yourself.
Kindly note: If you submit a portfolio of another student or one that is identical to that of
another student, you will be handed over to the University’s disciplinary committee.

Also, take note of the following:
1. Please submit a typed portfolio only. No handwritten portfolios will be accepted
2. Password-protected submissions will be given a zero mark.
3. Ensure that you submit the correct portfolio, as mistakes cannot be corrected later.
4. You must use the template provided. A portfolio written on a different template, or
a previous year’s template, will not be marked.
5. A portfolio with a fake school stamp will be handed over to the Legal Department
for investigation.
6. Do not cut and paste the school stamp or embed it. Stamp each page and scan
the pages into one PDF document.
7. You must do your teaching practice at a school registered with the Department of
Basic Education.
8. Please note that you will be penalised for omitting the above instructions. You will
not get more than 50% for intentionally ignoring the instructions to get a second
chance.
