Continuous quality improvement (CQI) is a philosophy and attitude that allows for the
analysis of capabilities and different processes, with the aim of achieving repeated
improvement that contributes to achieving patient satisfaction, specifically in issues
related to health care and patient satisfaction (Hill et al., 2020).
The applications of continuous quality improvement are multiple. An example could be
when a registered nurse provides care in the emergency department of a health unit. She
detects that a problem that has occurred continuously is the long waiting times required
for patients to be seen, which caused dissatisfaction and discontent among them. To
apply CQI in this scenario, the first step would be the timely identification of the
problem (long waiting times to be seen in the emergency department). This would be
followed by a detailed analysis of the problem, which requires the search for
information about the problem in order to understand the causes that gave rise to it.
Specific data could be identified, such as waiting time, the number of patients seen per
day, the number of staff available for care each day, and how their work time is
organized.
In this case, it is essential to analyze and develop an improvement plan that contributes
to increasing patient satisfaction. The implementation of a triage system would allow
patients to be prioritized according to the severity of their health problems. Adjusting
staff schedules is a fundamental element to consider in order to improve care coverage
during times of greatest influx of staff to the emergency department.
In order to implement this plan, it is necessary to train staff so that they properly
understand the new triage system, and how schedules will be adjusted to ensure faster
care without decreasing quality. This process also requires continuous monitoring of its
effectiveness, checking how the plan improves and optimizes the waiting time for care.
