Scenario

You are a member of an interdisciplinary team participating in patient rounds at the start of your shik.
You notice the physician charting that the patient is alert and oriented x3, but the patient was clearly
confused, which the physician acknowledged during rounds.

Discussion Question

How would you approach this scenario? Apply one of the ethical principles discussed in Dynamics of
Nursing: Art and Science of Professional Practice to this scenario. Discuss how organizational culture can
help manage errors.

cited using APA format. Responses to peers or faculty should be 100-150 words and include one

Class Resources, to understand the expectations for initial discussion question posts and participation
posts, respectively.




The ethical principles discussed are autonomy, beneficence, fidelity, justice,
nonmaleficence, privacy and confidentiality, respect, and veracity (Dean et. al, 2022). Veracity is
a main principle that is being violated. The physician is not being truthful regarding the
documentation of the patient’s condition. This lack of truthfulness could lead to a failure in the
ethical principle of nonmaleficence. As healthcare providers, we should do no harm yet charting
incorrectly can cause patient harm as it does not reflect an accurate picture. This inaccuracy
could delay care that this patient may need, thus leading to a poor outcome. In my current
practice, we are very close to patient providers. Often these providers look to the nurses charting
to help them write their surgery notes (op notes). However, in this situation, I would confront the
provider. I would begin by asking them if they had this patient confused with another. I would
then bring up how the provider acknowledged this patient was confused while in the room. The
physician could have just misread their own notes, so I would give them the benefit of the doubt.
I would make sure my own charting was correct regarding the patient’s condition. However, if
needed I would escalate the issue to leadership. Some methods organizations can help manage
these errors is by charting in real time. Many providers have scribes that follow them while
rounding to document in real time. Rounding in groups and discussing the patient all together
can help clarify actual situations. As such, there would be multiple notes from providers
detailing the patient’s correct situation. In the worst-case scenario if a provider would fight the
situation, many organizations have a way to escalate or call out this behavior. One of the hospital
systems I have worked at had a “Safety Always tool which taught nurses to: (1) practice a
questioning attitude and validate and verify, (2) check details and act with intention and focus to
avoid unintentional errors, (3) speak up and ask a question, voice a concern, and use a chain of
