SECTION 2: History of Present Illness (HPI)

Javier Flores is an 18-month-old male who presents with his mother due to a 2-to-
3-day history of a progressive "barking" cough that sounds like a seal. The mother
reports that the symptoms began as a mild upper respiratory tract infection with
low-grade fever ($100.4^\circ\text{F}$), clear nasal discharge, and congestion.
Over the past 24 hours, the cough became harsher and distinctly metallic/barking,
worsening significantly at night.

The mother notes that Javier makes a high-pitched, squeaky sound when
breathing in, particularly when he is crying or agitated. She reports mild mild
intercostal retractions when he cries, but states he breathes relatively
comfortably when calm at rest. There is no history of choking, sudden gasping, or
foreign body aspiration. He has maintained adequate oral fluid intake, though
slightly reduced from baseline, and continues to produce wet diapers (4–5 in the
last 24 hours). He has no known drug allergies, is up to date on all childhood
immunizations (including Hib and Pneumococcal), and has no prior history of
reactive airway disease or intubation.
