Patient Demographics
,Age: 83 years old
Sex: Female (assumed; you can change if needed)
Living Situation: Lives alone with occasional family support
Mobility Status: Independent with a walking stick; some difficulty with stairs
Reason for Encounter
Primary Complaint: Chronic right knee pain worsening over the past 2 months
Functional Impact: Difficulty walking, climbing stairs, getting out of chairs; reduced participation in social
activities due to pain
History of Presenting Complaint
Onset: Gradual onset over the last few years; significantly worse recently
Duration: Ongoing for several years, but exacerbated over the past 2 months
Character: Dull, aching pain with occasional sharp twinges
Location: Right knee, particularly around the medial aspect
Aggravating Factors: Walking, standing for long periods, cold weather, stairs
Relieving Factors: Rest, heat packs, over-the-counter analgesics (e.g., paracetamol)
, Severity: 6/10 at rest, 8/10 with activity
Past Medical History
Osteoarthritis (diagnosed 5 years ago)
Hypertension
Type 2 Diabetes Mellitus (well-managed with diet and metformin)
No previous knee surgery or major trauma
Medications
Paracetamol as needed
Metformin 500 mg BID
Amlodipine 5 mg daily
Occasional ibuprofen (with caution due to age and comorbidities)
