Brief Overview
This performance review pertains to Javier, a registered behavior technician. Despite
having been adequately trained to complete the training, Javier has not been able to complete the
session notes completely and appropriately. He also showed the capability to complete the notes,
but has constantly failed to complete them after the sessions as accurately and timely as
expected. The operational definition for the presenting problem is a behavioral deficit in
completing tasks promptly and accurately.
