2021). These obstacles have significantly impacted the evolution of efficient solutions that

promote more value-based care initiatives within these healthcare organizations.

One of the most significant downsides has been the lack of immediate concern for quality

care aims throughout the industry. Implementing new technology is critical for clinical and

financial success to transform the healthcare system from a fee-for-service to a value-based

reimbursement model. Paducah Health Medical System (PHMS) will require integrating a new

program that will effectively communicate with the current data analytics to foster clinical

efficiency, enhance patient health outcomes, and care quality, and support the community's

public health concerns.


Strategic Plan


Presenting a tailored, collaborative patient care approach will permit PHMS to integrate data

analytics that reshapes care delivery standards and enriches efficiency. Utilizing data analytics

allows clinicians to concentrate on top priority responsibilities like patient care. Supplemental

software tools can also support the improvement of patient quality and compliance. Including

PHMS's current HIT system, is choppy and unproductive, making it challenging to exchange

accurate patient health information. More importantly, the system is entirely inoperable for

several departments within the organization. These conditions have induced an ineptitude in

retrieving essential patient data and stagnating daily clinical operations. PHMS did implement an

external website to improve patient autonomy and buy-in. Unfortunately, the website's

limitations have resulted in minimal patient utilization. Patients have deemed the website

disjointed and impractical (Paducah Health Medical System Profile, 2021).

Enhancing the organization's interoperability is critical to improving health interactions

between clinicians and patients. The clarity obtained through interoperability lessens the

following population health matters (Health IT Playbook, 2020).

• Recurrent testing

• Medication administration errors

• Readmissions

Patient wait times are a substantial population health concern. They are generally affected by the

inability to correctly and cohesively review patient data within a timely manner. Due to the

severity of some patient conditions, health risks can present from erroneous lab values reported.

The new system will help to address the related population health concerns by safeguarding and

enhancing the PHMS community.

Converting PHMS to a value-based care delivery model is critical to improving the

surrounding community's population health, improving patient experience, and increasing cost

savings. Demanding high-quality care as a requirement for payment instead of the amount of

treatment provided is a culture transformation. Supporting interactions within an acceptable

timeframe is a critical component of the new HIT system. Likewise, enforcing a system with

billing and coding automation is essential. Data analytics and automation that can simulate

repetitious administrative tasks can reduce staff time inefficiently utilized. Predictive modeling
