Introduction
Encopresis is the repeated passage of stool into
inappropriate places such as clothing in children
older than four years who have achieved bowel
control. It is often associated with chronic
constipation and stool withholding behavior. This
case study presents a 7-year-old boy, Esteban Soto,
who was evaluated for frequent stool leakage and
chronic constipation.
Patient Information
, Esteban Soto is a 7-year-old boy brought in by his
mother due to involuntary stool leakage occurring
multiple times weekly. His symptoms started several
months ago and have been progressively impacting
his social and school life.
History of Present Illness
Esteban’s mother reports that he leaks stool in his
underwear about 3 to 4 times a week. He
experiences hard, infrequent stools approximately
every 3 to 4 days that are painful to pass. The
leakage usually occurs between bowel movements.
Esteban avoids using the bathroom at school due to
embarrassment and anxiety.
Past Medical History
Esteban has a history of chronic constipation since
the age of 3 years. He was late to achieve bowel
control but has no history of surgeries or
hospitalizations. His immunizations are up to date,
and there are no known drug allergies.
Social History
He lives with his parents and younger sibling. His
diet is low in fiber, and he tends to consume few
